The first job is understanding what the business does, who it is for and what the website needs to achieve. Then a real concept is designed, usually the homepage in full, so you are reacting to something concrete. For redesigns, what already works is kept and what holds the business back is rebuilt.
Sites are built with modern development tools rather than page-builder templates, which is why they are fast and why every one can look different. Where a business needs to run a shop or edit its own content, it is built on a platform such as Shopify or Squarespace, and designed so it still looks like nobody else.
